The Maryland Lifetime Excellence in Family Medicine Award recognizes individuals not only for their achievements, but also for the inspiration they provide others.

1. Active or Life Member of MDAFP
2. Served in clinic/community more than 20 years
3. Established impactful, positive mentorship to colleagues or supportive practice partner
4. Demonstrated competence and professionalism throughout their career
5. Had a significant impact on their community, through clinical services, teaching, research, volunteer work, or other community involvement
