Patricia A. Czapp, M.D.* ,
Dr. Czapp serves as Chair of Clinical Integration at Anne Arundel Medical Center and Chair of AAMC’s Collaborative Care Network, a Medicare Shared Savings Program Accountable Care Organization. In both of these roles, she promotes population health improvement by engaging patients, specialty and primary care physicians, hospitals, and community-based organizations in chronic disease management, readmission reduction, and disease prevention programs.
Pat provides patient care at a Health Enterprise Zone community health center in Annapolis which she developed in collaboration with public housing, hospital and government resources. She has served as Chair of the American Academy of Family Physicians’ Commission on Health of the Public and Science and she is active in state task forces and committees that focus on physician alignment and engagement in Maryland’s All Payer Model.
She earned her undergraduate degree in biological anthropology and zoology at University of Michigan and her MD at Georgetown University, where she completed her residency in family medicine and served as Chief Resident. Pat and her husband Christopher Lynnes, a NASA data systems architect, live with their dog Victor in Annapolis, MD
